Responsibilities
Tasks
Personal Support & Care

Assist clients with activities of daily living such as bathing, grooming, toileting, dressing, mobility, and personal hygiene.

Provide companionship and emotional support to promote client comfort and wellbeing.

Support safe transfers, positioning, and the use of mobility aids.

Assist with feeding and meal support, including preparing meals based on dietary needs.

Observe and report any changes in physical, mental, or emotional condition to the supervising nurse or care manager.
Health & Safety Support

Assist with medication reminders according to care plan and under the direction of a nurse or supervisor.

Perform routine health-related tasks (e.g., non-sterile dressing changes, basic skin care) under supervision.

Maintain safe and clean living environment, including light housekeeping related to client care (laundry, dishes, bed-making).
Light Coordination Duties (Non-Supervisory)

Notify clients or families of staff changes or appointment confirmations.

Answer or return routine phone calls and messages from clients or staff.

Record shift confirmations or cancellations in client and staff logs.
-
Keep contact lists and availability records up to date.
-
Support onboarding by preparing orientation materials or checklists.
-
Assist with confirming client address, phone number, or care notes for scheduling accuracy.

Maintain polite and professional communication when representing the agency.
