Work must be completed at the physical location. There is no option to work remotely.
Responsibilities
Tasks
- Attend report from supervising RN.
- Note all of the residents that require monitoring: bowel protocol, infections, post fall, palliative,
significant change.
- Ensure assignment sheet is collected and any residents that require additional monitoring are
highlighted.
- Identify which residents are on bath days.
- Collaborate with team to decide what residents will be getting up first and who will be completing what tasks (independents, doubles, baths, etc).
- Complete rounds on residents to ensure they are sleeping and that they do not require
assistance.
- Assist residents with toileting in accordance to their care plan (brief changes, bed pans, lifts,
commodes).
- Answer call bells as required and assist residents with activities of daily living.
- Complete frequent rounding for listening to bed alarms, residents calling for help, requiring
assistance, wandering, etc.
- Document tasks into POC in a time sensitive manner (point of care).
- Review care plans to ensure that proper care is being completed and any changes are reported
to the supervisor RN to be recorded and updated.
- Restock supplies on cart and in supply rooms for other shifts.
- Communicate any significant changes to nurse supervisor to be documented and followed up.
- Apply creams as delegated by registered staff during care rounds.
- Ensure that residents who are high falls risk or increased monitoring have the appropriate interventions set up as per care plan (PPE stocked caddy, fall mat, bed/chair alarm, bed lowered, PASD applied, etc.).
- Ensure all resident items are labelled appropriately and any items that are not or need repair are reported to the supervising nurse (glasses, hearing aids, dentures, clothing, etc.).
- Restock briefs for the day, caddy's when on outbreak, supply rooms when low, and remove all cups, snacks and clutter from resident rooms.
- Clean resident's assistive devices as per schedule, and allow time to dry.
- Document on all required sections of POC and ensure no residents are left in red.
- Ensure all required information is passed on to nurse supervisor.
- Wait until PSW are finished report before leaving the floor.
